Insurance Policy

Massage Square offers direct billing to various Insurance Providers. In order for us to do this service for your convenience, we require a valid Credit Card, or Visa Debit on file for any charges not covered by your Insurnace Provider. This information will be collected at your first visit with us.

Rest assures your payment information on file will not be charged for the outstanding balance until your insurance provider has completed your claim and responded to
Massage Square with the outstanding amount. Our lovely staff will then contact you letting you know of the remaining amount not covered and obtain your authorization with your preferred method of payment to complete the transaction at your convenience.

Some insurance providers require a Physician’s Referral for Massage Therapy. Kindly contact your insurance provider to find out their specific document requirement. Please allow your Insurance Provider adequate processing time after your doctor’s note or referral submission prior to your treatment date.

If you have more than one insurance provider, please note that the primary insurance may not allow us to claim certain secondary insurances. Client additional document and confirmation maybe required. In this case, you will need to submit your remaining balance to insurance provider yourself. We thank you in advance for your understanding.
